Card Replacement

Please read ALL the instructions, We will contact you when your request has been received.

1. Replacement cost for a course completion card is $5.00 PER CARD + $2.00 research/postage fee.
2. Payment is made via PAYPAL, charged to your credit card.
3. ALL requested information MUST be supplied or your request cannot be honored.
4. Any fraudulent attempt get us to issue a card for a course you did not complete with us will result in the loss of your payment charged to your credit card, AND THERE ARE NO REFUNDS. WE WILL NOT TOLERATE FRAUDULENT REQUESTS!
5. Requests will be handled the same business day, and will be processed and mailed on the next business day.
6. If you need a replacement for more than one card, be sure to include that on the payment form. Replacement cards will not be sent if the fee has not been paid. (If you need a replacement for both a CPR and a First aid card, be sure that you pay for 2 cards.)
7. After you have read all of the above instructions click on the PAYPAL link below. That will take you the paypal site, then to the information request form to enter your information.
